About HealthBridge
HealthBridge is redefining how hospitals, healthcare systems, families, and post-acute providers navigate care transitions for older adults.
Built through the success of our hospital partnerships in Portland and expanding nationally, HealthBridge serves as a strategic bridge between hospitals, patients, families, and community-based senior care solutions. Our model supports discharge planning, post-acute navigation, patient and family guidance, and operational coordination designed to improve outcomes, reduce friction, and enhance the patient transition experience.
As we expand nationally, we are transitioning from a relationship-driven pilot model into a scalable healthcare services platform with standardized operational delivery, measurable outcomes, and enterprise-ready infrastructure.
We are seeking an experienced healthcare operations leader to help architect and operationalize that future.
