The Role: Connector. Collaborator. Catalyst.
You’ll be the face of Oasis in the Twin Cities healthcare community. Your job is to build and deepen referral relationships across the care continuum — making sure that when a patient or family needs placement support, Oasis is the first call their care team makes.
 
Key Responsibilities
  • Build and maintain referral relationships with hospital discharge teams, skilled nursing facilities, home health agencies, hospice providers, elder law attorneys, and other aging-focused professionals.
  • Serve as a trusted resource for care teams navigating complex transitions — someone they call because they know you’ll get it right.
  • Identify and pursue new outreach opportunities across the Twin Cities market.
  • Represent Oasis at community events, healthcare settings, and professional networks.
  • Collaborate with our advisors to ensure seamless handoffs and strong family outcomes.
 
Who We’re Looking For
This role is built for someone who has spent time in the field — not just near it. If you’ve worked in hospice, home health, senior living, or hospital-based care transitions and spent meaningful time building relationships with referral partners, you’ll feel right at home here.
 
  • 2+ years in a business development, liaison, or community outreach role within hospice, home health, senior living, or a hospital/SNF setting.
  • Existing relationships across the Twin Cities healthcare landscape — you know the players, the facilities, and how care transitions actually work.
  • Deep understanding of the continuum of care: from acute and post-acute through long-term placement and end-of-life support.
  • A natural relationship-builder who earns trust quickly, follows through consistently, and communicates with clarity and warmth.
  • Self-directed and comfortable managing your own schedule, pipeline, and territory.
  • Must be located in the greater Minneapolis/St. Paul area.
 
What We Offer
  • Competitive base compensation + performance-based incentives with compensation totaling between $70,000 - $150,000.
